New Story Behavioral Health (NSBH) is a group mental health practice committed to providing compassionate, holistic psychiatric care and psychotherapy. We offer services through secure in-person and electronic methods.

Protected Health Information (PHI) refers to any health information that identifies you and relates to your past, present, or future physical or mental health condition, the care you’ve received, or payment for that care. 

How We Use and Share Your Information 

– Treatment: To coordinate your care among NSBH clinicians. 
– Payment: To bill for services and provide documentation related to insurance claims. 
– Health Care Operations: For internal management, quality improvement, and compliance. 
– As Required by Law: For legal or regulatory requirements. 
– With Your Authorization: We will obtain your written permission for any other use.

Communication Methods and Associated Risks 

Text Messaging (SMS): 
NSBH may send appointment-related text messages. By opting in, you acknowledge that SMS is not a secure method. To opt-out, reply STOP. 
 
Email: 
We use secure systems like TheraNest for reminders. Unencrypted email communication carries risks. By initiating email contact, you accept these risks. 

Data Collection and Protection 

Forms submitted through our website are powered by JotForm, a HIPAA-compliant platform. We collect basic contact and health information to follow up on care requests. 
 
Our website is hosted on Wix.com, which may collect general analytics. These cookies do not collect personal health information. 
 
We use encryption, secure servers, and access controls to protect your data. 

Third Parties and Business Associates 

We work with trusted service providers (e.g., Wix, JotForm, TheraNest) under Business Associate Agreements (BAAs) to ensure they protect your information in accordance with HIPAA. 

Data Retention and Changes 

We retain client health records for at least 7 years, in compliance with applicable laws.
